It's a guy with schizophrenia (a guess) that's spiraling downward into depression and despair. Most people here including me believe that Firth did a awesome job at creating a disturbing but emotional atmosphere around the main character. Firth also showed the audience how the main character perceived the world around him (dark, dreary and depressing; everyone laughing at you, wanting you dead, no escape from his illness, etc). It's certainly my favorite flash cartoon by David.
